  • HVEHF is no longer the provider for our Geriatric HCHV contract in San Jose. (we have not identified another provider at this time.)

For those whom may be new to VAPAHCS, the HCHV and GPD program are our homeless shelter and transitional shelter housing programs that we have agreements with.  Provider can call or Veterans can self refer. If you are having any difficulty getting a Veteran into one of the HCHV/GPD programs, please reach out to me to see if I can better assist you.

SSVF- Deposit and Rent Assistance

SSVF(Supportive Services for Veteran Families) Provide deposit and rent assistance to eligible homeless or at risk of homelessness veterans. Grants are issued through the Veterans Resource Center and the Homeless Services Center.

Hud-Vash (Permanent Supportive Housing)

The HUD-VASH Program is a partnership between the Department of Housing and Urban Development (HUD), the Department of Veterans Affairs (VA), and many local Public Housing Authorities (PHA). The program combines the Housing Choice Voucher providing rental assistance for homeless veterans with case management and clinical services provided by the VA. The Housing Choice Voucher can be used at the the rental of their choosing.

Swords to Plowshare

We are a community-based not-for-profit 501(c)(3) organization. We are dedicated to supporting nearly 3,000 homeless, low-income and at-risk veterans in the Bay Area every year.  We offer employment and job training, supportive housing programs, permanent housing placement, counseling and case management, and legal services.

HCHV-90 Day Emergency Program

Healthcare for Homeless Veterans (HCHV) program provides emergency housing for homeless veterans for up to 90 days. Additionally it is a hub for housing and other services that allows the VA to reach and assist homeless Veterans by offering them entry to VA Healthcare and other resources.

GPD-2yr Transitional Housing

Grant and Per Diem Programs (GPD) provide transitional housing for Veterans up to two years. They all help facilitate veterans and VA healthcare and resources. 

Veterans Transition Center (VTC) and Veteran Housing Facility (VHF) are both part of the GPD program. They are committed to providing two years transitional housing and support services for homeless Veterans.

Housing Resource Guide

The Housing Resource Group connects Veterans with local housing resources. The guide below lists all of the meeting times and locations. Low-income Veterans who are eligible for HUD-VASH receive a specialized Housing Choice Voucher to be used at the rental of their choosing.

CALVET Homes

​The Veterans Homes of California offer affordable long-term care to aged and disabled veterans as well as their eligible spouses and domestic partners. VA helps Servicemembers, Veterans, and eligible surviving spouses become homeowners. As part of our mission to serve you, we provide a home loan guaranty benefit and other housing-related programs to help you buy, build, repair, retain, or adapt a home for your own personal occupancy.
